Participant Profile and Commitment Form
This workshop is a year-long, community-centered writing program that builds connection and trust through ongoing participation. Because this program is intentionally small, we ask participants to commit to engaging with the program over time, to the best of their abilities. This form serves two purposes:
To confirm your intention to participate.
To collect some information required by our grant funder.
Your responses are confidential and will only be shared in aggregate (group) form for reporting purposes. Individual responses are never published or attributed to you.
Your participation helps BIASC continue offering programs like Every Story Matters at no cost.
